The CBTF Contingent Hotel Worksheet and Payment Forms are now available.

A document and information checklist is also included.

Questions about the Contingent Package or the Package Payment Forms should be directed to the WC Contingent Coordinator at presidentcbtf [dot] ca.

All forms (Hotel & Payment Worksheets, Athlete Information Sheet, Code of Conduct, Secret Pal, and Health Certificate), and payment, can be submitted at any time during the Team Trials or at the Contingent Meeting on May 22.

Also, we remind travelers that if their child is under the age of majority, a travel consent form (if travelling with no parents or one parent) and a chaperone form (if travelling with no parents) are required. These are available on request (presidentcbtf [dot] ca) if required.
